Short Term Care & Post Acute

Creating an individualized PASSPORT to Recovery allows you, our patient, to focus on returning home after surgery, serious injury and illness. Our staff Physiatrist and Medical Director CRISTA Rehab & Skilled Care, Charlotte H. Smith MD, is an expert in senior care and well respected in rehabilitation medicine. We have on-site physicians, including geriatric specialists, and also employ a team of physical (PT), occupational (OT), and speech (SLP) therapists with over 175 years of collective experience. Our nursing staff and caregivers play a significant role in your road to recovery and are trained and certified in rehabilitation nursing.
